We appreciate any suggestions about how to improve out service. You can provide up with feedback by filling in a feedback form in the waiting room, emailing firstname.lastname@example.org or call (02) 72005522 and ask to speak to the practice manager.
A phone interpreter can be provided if necessary. Please advise the language spoken and preferably give us a few days notice to organise an interpreter.
Eternal Doctors is committed to preventative healthcare. We may send reminders on occasion to offer you relevant preventative health services.
An appointment to see a doctor must be made in order to be issued a medical certificate. Certificates cannot be backdated.
We request that you attend for an appointment for a repeat prescription. If you have seen your doctor within the last three months you may be able to request it online for some medications. A fee of $20 will apply which is not refunded under Medicare. Two business days notice must be given for all prescription requests.
We recommend that all patients make a follow up appointment to discuss the results of any tests including blood tests and x-rays.
There is ample parking available on Frenchmans Rd and surrounding streets. Please pay attention to street signs as some areas are timed. The off-street parking adjacent to the practice is for the customers of Chambers cellars and the fruit and vegetable store only. We apologise for any inconvenience but ask you to use on-street parking only.
If you have seen your doctor within the last three months and need a repeat referral to someone you have seen previously you may be able to request one from your doctor online. A fee of $20 will apply which is not refunded under Medicare.
